How inova health is customizing the billing process to improve patient experience.

As hospitals and health systems work to optimize patient revenue management and payer-focused billing processes, one Northern Virginia-area health system is turning to VisitPay, an online financial engagement platform that aims to simplify the billing experience for both patients and health systems.

In a March 1 webinar sponsored by VisitPay and hosted by  Becker's Hospital Review , Wendy Alexander, vice president of analytics and client services at VisitPay, joined Liz Ward, vice president of revenue cycle at Falls Church, Va.-based Inova Health System, and Drew Cobb, vice president of clinical applications at Inova, to discuss how VisitPay addresses patient needs, why Inova chose VisitPay and the health system's experience implementing the platform.

VisitPay's platform is customizable to each health system based on the segments and subsegments of the patient populations they serve, and helps health systems across the country improve patient and provider outcomes by boosting patient satisfaction, increasing payment rates and reducing cost to collect. VisitPay clients include Salt Lake City-based Intermountain Healthcare and Boise, Idaho-based St. Luke's Health System.

Inova, a six-hospital health system serving 2 million-plus patients annually, worked closely with VisitPay to evaluate patient billing process issues and develop a successful solution. Inova already uses VisitPay analytics, a patient-focused technology that helps the health system understand how their patients tend to pay. The technology enables Inova to tailor their call strategies inside the call center. For example, these tailored interactions help Inova know how and when to be proactive with consumers.

Now, Inova is implementing VisitPay for patient payments. The payment system not only allows patients to pay online, but also gives Inova a cost-effective digital platform to enable these consumer finance strategies. The pay service is set to go live soon.

Tailoring the revenue cycle to fit patient needs

VisitPay's platform helps Inova's patients better understand their bills and payment processes by allowing them to access information about their visits and payment status either online or by talking with an Inova representative, and by presenting payment options that meet their needs.

By giving patients more engagement options, the digital platform furthers the health system's goal of reaching patients on a more individual level. "Patients are looking for a digital solution," Ms. Ward said. "In fact, many patients would prefer only to interact with us in a digital way and many patients want to have the flexibility of both." In this way, VisitPay allows Inova to function more efficiently and effectively than the slower process of paper statements and dependence on the call center for all tailored interactions.

Ms. Ward discussed a recent experience she had with a patient who said for him to understand what was happening with his account at Inova, he had to have three pieces of paper: Inova's statements, the payer's explanation of benefits and a statement from an electronic payment portal. "With the information and the tools we have now, we can … keep all their payment information in one place."

Managing long-term patient financial plans under a digital platform

While a digital platform allows a health system to provide more flexible payment options to patients, there are potential challenges for health systems when managing complex, long-term patient financial plans.  

Some patients require monthly payment plans to afford costly procedures. Health systems are challenged to find a balance between giving patients who need it more time to pay and making sure the organizations continues to collect payments. "We are making a tradeoff — the accounts receivable will increase. We have to balance that to ensure we provide the longer-term financing for those who need it but also that those who can pay do so," said Ms. Ward.

VisitPay facilitates flexible payment plans to manage long-term financing for surgical patients as well as short-term payment plans for medical office visits. Inova will soon complete an internal launch for employees to help test VisitPay's platform before they fully deploy the system for their patients.

Implementing a digital platform that adjusts to patient preferences  

Implementing a new technology from an outside vendor is a challenge for any organization, particularly for hospitals and health systems, Mr. Cobb said.

In healthcare, new technology is particularly challenging because the industry has not typically been an early adopter of new technology, Mr. Cobb added. As a result, technology vendors from a different industry might expect healthcare organizations to have tools they might not have.

Another challenge for adopting new technology platforms stems from the significant differences between patient populations at each hospital. "There's not a cookie cutter for patient populations," Mr. Cobb said. "You have to be able to adjust the outline for the system depending on the demands of each healthcare customer. That's the learning curve for any vendor that comes into this industry from another industry."

Therefore, to adjust to different patient populations' needs, VisitPay allows Inova to continually test different consumer finance experiences and explore payment options to learn what works best for their patients, all without coding or interrupting the EMR system. "All patients — whether they manage payments through technology platforms or by phone — must have access to the same payment options," Mr. Cobb said.            

Implementing an online engagement platform can help health systems bridge the technology gap for patient interaction. "Patients use technology in other parts of their life and they hit healthcare and they wonder why they don't have that same portability, or the ability to go to one place for all the answers," Mr. Cobb said. "We want our patients to have all their financing information all in one place, and that's a critical piece of making payment information more user-friendly."

To support your health and wellness, Inova provides affordable, comprehensive benefit coverage that helps you and your family stay well and pay for care if you get sick or injured.

Our benefits program includes two medical plan options with different coverage levels and costs, so you can choose the one that’s best for you. These plans are administered by Innovation Health, a partnership between Inova and Aetna. If you live in the following counties or cities in Virginia: Arlington, Fairfax, Prince William, Loudoun, City of Alexandria, Fairfax City, Falls Church City, Manassas Park City, and Manassas City you are eligible for the Innovation Health HSA HDP (High Deductible Plan) or Innovation Health PPO plans. If you live elsewhere, you are eligible for the Outside NOVA HSA HDP or Outside NOVA PPO plans, which have similar coverage levels.

You can enroll as a new hire, during Annual Enrollment, or if you become benefits eligible due to a job change within Inova or if you have a qualifying life event . To see your contributions and enroll, log in to the Inova Benefits Center website.

With our medical plan options, you have access to two levels of in-network care: Maximum Savings and Standard Savings. The Maximum Savings tier includes Inova facilities and providers, as well as Signature Partners, Valley Health, Privia, Loudoun Medical Group and Aetna providers in geographic areas not represented by the aforementioned groups. When you choose an in-network provider in the Maximum Savings tier, you'll pay the least. Look for the Maximum Savings Label in the Directory of health care professionals for Inova team members .

The Innovation Health HSA HDP (High Deductible Plan) and Outside NOVA HSA HDP plans pair low-contributions, high-deductible coverage with a tax-free Health Savings Account (HSA) that helps you save money and plan ahead for future medical expenses. This combination gives you more control over your money and rewards you for making cost-conscious choices.

Note: Basic Hospital Indemnity insurance is provided at no additional cost if you enroll in the Innovation Health HSA HDP (High Deductible Plan) or Outside NOVA HSA HDP.

How the Innovation Health HSA HDP (High Deductible Plan) and Outside NOVA HSA HDP plans work

You pay contributions toward your medical coverage.


You can set aside tax-free money from your paycheck from your paycheck to help cover your costs — now, or in the future.


You pay 100% of your medical and prescription costs until you meet the annual deductible .



After meeting the deductible, you and the plan share the cost of covered medical care and prescriptions, with the plan paying the majority.


Out-of-Pocket Maximum

You’re protected by an annual limit on costs — the plan pays 100% of any further covered expenses for the rest of the year.

Keep in mind: You pay nothing for in-network preventive care — it’s covered in full.


Use your HSA to save money and plan ahead!

Contributing to your HSA is a great way to budget for deductibles and other out-of-pocket expenses while also saving money — your HSA contributions are tax-free!*

Keep in mind:

  • The Innovation Health HSA HDP (High Deductible Plan) and the Outside NOVA HSA HDP cost you less from your paycheck , so you may have extra money available to put in your HSA.
  • You can only spend HSA money that’s actually been deposited into your account. If you don’t have enough money in your HSA when you need it, you can pay another way and reimburse yourself later so you take full advantage of your HSA’s tax savings.
  • You will never forfeit any money left in your HSA — it rolls over year after year. If you know about future expenses or want to save for your health care costs in retirement, set aside a little extra each paycheck so your balance can grow over time.
  • You can change your HSA contribution amount throughout the year as needed to keep up with any changes in your situation.
  • You can pair your HSA with a tax-free Limited Healthcare Flexible Spending Account (FSA) to save even more.
  • Inova pays the monthly administrative fees for you.

*Contributions are not subject to federal income tax. However, HSA contributions are currently subject to state income tax in CA and NJ. Consult with your tax advisor to understand the potential tax implications of enrolling in an HSA. Money in an HSA can be withdrawn tax-free as long as it is used to pay for qualified health-related expenses. If money is used for ineligible expenses, you will pay ordinary income tax on the amount withdrawn, plus a 20% penalty tax if you withdraw the money before age 65.

The Innovation Health PPO or Outside NOVA PPO offer slightly lower out-of-pocket costs in exchange for higher contributions. With this plan, your costs are more predictable, but you’ll likely still have out-of-pocket expenses.

How the Innovation Health PPO and Outside NOVA PPO work


You pay a small fee at the time of service for doctor visits and prescriptions. (Medical and prescription drug copays do not count toward your deductible.)

For care that doesn’t charge a copay, such as physical therapy, you pay 100% of the costs until you meet the annual deductible .

After meeting the deductible, you and the plan share the cost of certain services, with the plan paying the majority.


Save money with an FSA!

A Healthcare Flexible Spending Account (FSA) lets you take advantage of tax-free savings when paying for health care. Be sure to plan your FSA contributions carefully: you can only carry over up to $610 of unused money in your FSA to the next year; you will forfeit any remaining amount above $610.

Gender Affirmation Care Benefits

The Inova medical plans through Aetna provide gender affirmation care benefits and services available to support your decision to transition to your affirming gender. These benefits can help with the cost of gender reassignment surgery and associated procedures and services. Nurse Concierge Program

Clinical care management staff from the Innovation Health Nurse Concierge Program may reach out to members upon discharge from an inpatient setting or emergency room admission, and provide education, help with follow-up appointments and service referral assistance. Call 571-421-2810 , Monday through Friday between 9:00 a.m-4:30 p.m. if you need help getting medical care.

Informed Health® Line

The 24-hour nurse line is available at no cost to you only if you’re enrolled in an Innovation Health medical plan. When you call 1-800-556-1555 , you’ll have access to registered nurses who can provide information on a variety of health topics. You can get answers to health questions, learn more about getting and staying healthy, and communicate more effectively with healthcare providers.

Behavioral Health

We all need a little help at times — whether you’re facing a mental health condition or caring for someone who is. Aetna Behavioral Health is available to team members enrolled in the Innovation Health/Aetna medical plan. This program includes resources and support to help you get effective treatment — whatever your needs may be. You can schedule phone calls or virtual visits with your provider in the Innovation Health/Aetna network.

Finding a Primary Care Physician

Inova’s board-certified adult and pediatric Primary Care Physicians (PCPs) are in-network and have the Maximum Savings label in the Directory of health care professionals for Inova team members . They offer:

Save with GoHealth Urgent Care Facilities

Save money by using GoHealth urgent care facilities, an Inova partner. Team Members enrolled in the PPO plan will pay a $5 copay when using a GoHealth urgent care facility. The copay for all other urgent care facilities is $35 per visit.

At GoHealth facilities, their highly credentialed healthcare professionals are ready to handle everything from runny noses and coughs to sprains and cuts, asthma attacks, fevers and much more. When you carefully consider the financial impact of your health care choices, you can help lower costs for yourself and the entire company. Get the most from your medical benefits by following these tips to be well and buy smart:

  • Use in-network providers. To find a PCP with maximum savings, log in to Innovation Health/Aetna and click “Find Care.” You’ll find the best providers and facilities under the “Maximum Savings” indicator for each provider listing.

  • Use tax-free money to pay for eligible health expenses. Contributing to a Health Savings Account (HSA) and/or a Flexible Spending Account (FSA) is easy and saves you money. You can set aside pre-tax dollars from your paycheck to use for your out-of-pocket costs. Keep in mind that with an HSA, you can only spend contributions that have actually been deposited into your account. With an FSA, you can only carry over up to $610 of unused money in your FSA to the next year; you will forfeit amounts above $610.

Financial Services (Before Your Day of Surgery)

Before your procedure, our Financial Services team will assist you with:

  • Verification of coverage and benefits
  • Obtain any required authorization, as needed
  • Assist you in understanding any expected financial liability
  • Assist with setting up any financial arrangements

We will request payment of deductibles and coinsurance/copays, either before or at the time of service. We will make every effort to contact you before your surgery, to let you know what your financial liability may be. Payment arrangement or applying for financial assistance can be discussed during this time. If we are unable to reach you, you should be prepared to pay any expected liability on the day of surgery.

Understanding Your Bill and Fees (After Your Day of Surgery)

You may expect after a procedure/ surgery that all surgical care providers will first bill your insurance company for the costs associated with the procedure/ surgery. You may then receive a bill for any remaining balance. For example, a remaining balance may be a deductible cost, co-pay, co-insurance, etc. after notice from your insurance company. You may receive separate bills from different providers within the surgical team.

You will be charged a facility fee for all services provided by Inova Woodburn Surgery Center, as we are an off-campus outpatient hospital department. This fee does not include fees for any of the following services that may be incurred during your stay at our facility, and you will receive separate billings for these services:

  • Anesthesiologist
  • Pathologist
  • Radiologist
  • Laboratory tests

Hospital Bill

A facility bill from the hospital or surgery center at which the procedure or surgery is performed. This bill will reflect expenses for staff, supplies and equipment used for the surgical services provided.

Surgeon's Bill

This is a separate bill for the surgeon’s services.

Anesthesiologist’s Bill

This is a bill for anesthesia services you receive during a procedure/ surgery and are provided by an anesthesiologist with Fairfax Anesthesiology Associates.

Pathology Bill

This is a bill for pathology services you receive for a procedure/surgery and are provided by a pathologist from Inova Central Lab.

Radiology Bill

This is a bill for radiology services you receive for a procedure/surgery and are provided by a radiologist from Fairfax Radiological Consultants (FRC).
